A Financial Affidavit is a document through which an individual person (the affiant) swears upon their present financial status, by providing explicit details about their monthly cash flow. Often, these documents are used in other cases, such as family matters, or for the opening of new credit or loans.

A Financial Affidavit contains all of the information required to create a full picture of the affiant's finances:

1. Full legal name;

2. Date of birth;

3. Full and current address;

4. Employment information;

5. Monthly income;

6. Monthly deductions;

7. Monthly expenses;

8. Assets;

9. A document name and if applicable, number verifying identity of the affiant; and

10. Reason the Affidavit is needed.

 

How to use this document

This document can be completed by any individual needing a form to verify their financial status. The affiant will be asked for detailed information regarding their monthly income, deductions, and expenses, as well as a valuation on each of their assets.

This document is easy to fill out and contains all of the information needed for a complete Financial Affidavit.

Once it is filled out, the Affidavit must be notarized, which means the affiant signs before a Notary Public, after providing valid legal identification.
