An Affidavit of Loss is a document that is used when someone has lost an item and needs to swear to the circumstances of its loss. Usually, Affidavits like these are used in situations where the affiant (this is the person swearing to the document) needs a replacement for the lost item, but it is not a necessity. They can also be used if something of great monetary value was lost and the affiant requires a legal document attesting to it (such as for insurance purposes).

Affidavits of Loss are very straightforward and simple documents.

They contain only the following details:

1. Full legal name of the affiant;

2. Date of birth of the affiant;

3. Address of the affiant;

4. Identifying details of the lost item;

5. The date of loss or discovery of loss;

6. An explanation of the circumstances surrounding the loss; and

7. The signature and notarization.

 

How to use this document

This document can be completed by any person who has lost an item, whether it is a document or a physical object, and needs to swear to its loss. It can be completed by the owner of the lost item, someone who had lawful possession of the item at the time of its loss, or someone who was charged with the safekeeping of the item.

Often, these Affidavits are used when official documents are lost, like Driver's Licenses or Passports.

This document is easy to fill out and contains the basic information needed for a complete Affidavit of Loss, regardless of the document or object that was lost.

Once it is filled out, the Affidavit must be notarized, which means the affiant signs before a Notary Public, after providing valid legal identification.
