Dear ________,

RE: Meeting on Sickness Absence

I am writing this letter to invite you to a meeting to discuss the reasons for your sickness absence for the period of ________ to ________. This meeting is set to take place at ________ on ________. The venue of this meeting will be at our office address.

We wish to discuss when you will be likely to resume work.

The medical report you submitted, will be discussed at the meeting. For your ease of reference, the medical report explained that:

________

It is advisable for you to look through the medical report prior to attending the meeting. In particular you may wish to consider whether there is any information within that report which you would like to comment on or do not agree with.

At the sickness absence meeting, we will look to discuss the following issues:

- If we can reach an agreement on a time frame within which you should return to work, including whether this return will be phased. A phased return to work would mean that although you have resumed back to work, you will not do carry out your full duties and/or hours initially. However, we would look to build up to your full duties and/or hours over an agreed period.

It is possible that we consider it premature to agree on a timeframe for you to return to work. If this is the case, it may be necessary to review your health status and any additional issues in a further meeting with you. We encourage you to inform us about any other matters that you may wish to address, either before the meeting takes place or at the meeting itself.

We will conduct this meeting in line with our Sickness Absence Policy, which can be found in the Staff Handbook.

The person handling this meeting will be ________.

You are allowed to bring a colleague to the meeting. If you intend to come along with a companion, please let me know the name of your companion as soon as possible.
