This document should be used for domestic travel within the Philippines and is requested due to the rising concerns about child abduction and trafficking. Authorities, such as law enforcement officers, may ask for proof of consent when a minor is traveling with one parent or another adult, such as a teacher, group leader, grandparent, or house helper.

The parents or legal guardians who are allowing the child to travel should affix their signature at the end of the document.
