A Travel Consent Form serves as a notification to relevant parties such as the authorities or any person concerned that a parent or legal guardian has granted permission for a child to travel without their supervision.

This document is necessary in the following cases:

  • when the child is traveling alone, as in the case where the child is nearing the age of majority or around the ages 15 to 17 years old and he/she is permitted to travel without adult supervision.
  • when the child is accompanied by only one parent/guardian, and the consent of the other parent/guardian is given to allow the child to travel with the accompanying parent/guardian.
  • when the child is accompanied by an authorized person under the authority of an organization or group.
  • when the child is accompanied by an authorized person who is an adult and is given authority to travel with said child.

This document should be used for domestic travel within the Philippines and is requested due to the rising concerns about child abduction and trafficking. Authorities, such as law enforcement officers, may ask for proof of consent when a minor is traveling with one parent or another adult, such as a teacher, group leader, grandparent, or house helper.

If the child is traveling abroad or internationally, an Affidavit of Support and Consent should be utilized for that purpose.


How to use this document

The use of this Travel Consent Form enables a parent or guardian to include the following information:

  • the names of the traveling children
  • their personal information
  • the purpose of the trip
  • who they will be traveling with
  • the planned travel route,
  • and the date of the travel up to its end
  • the children's health issues to assist in the proper caring of the children during the course of the travel
  • the contact details for accompanying adults who are in charge of the travel, if applicable, and the parents or guardians in case of an emergency.

It is advisable for children who are traveling to have a copy of the Travel Consent Form with them and for adult chaperones to carry a copy of the form to present to authorities when necessary. The parents or legal guardians who are allowing the child to travel should affix their signature at the end of the document.


Applicable law

Although not specifically required by law, this document may be presented to law enforcement officers or persons in authority to prove that there is no violation of R.A. 7610 or the Anti-Child Abuse Law. The information in this document when presented to said officers, will provide the necessary information such as the contact details to ensure that there has been no trafficking, abuse, or harassment committed against the child.
