AFFIDAVIT OF CHILD SUPPORT

2. I hereby promise and undertake to support by providing the needs of the child's psychological, emotional, moral and social well-being, as well as love, support, and giving the child the time and comfort I can provide.
3. To be specific, the support that I will provide to the child involves the payment of a sum of money to answer for the following expenses:

2. Shelter,
3. Clothing,
4. Medical Attendance, and
5. Education and transportation;
4. In case of extra-ordinary expenses including but not limited to hospitalization, are incurred by the child, I will also answer for the same, provided that there is proof that satisfactorily shows that such expenses are incurred for the well-being, health and safety of the child.
5. In order to provide the support needed by the child as specified in the preceding articles, I promise and undertake to send money to the child's mother, ________, in the amount of ₱________ (________ pesos) monthly.
