The Affidavit of Travel and Consent will provide that:

Choose the purpose of the Affidavit of Travel and Consent. Note that the proper situation must be chosen carefully as choosing the wrong option may result in the denial of passport, visa, or travel application.

"Child" only refers to a minor child. If the child will be accompanied by a person other than the parents (or the mother if the child is illegitimate), said person will be referred to as the "representative".

Choose "A child will apply for a passport accompanied by one or both of their parents (or the mother if the child is illegitimate)" if the child will apply for a passport with one or both of his parents. Note that an illegitimate child means that his parents are not married.
Choose "A representative will accompany the child in travelling abroad" if the representative will accompany them in traveling abroad. In this case, the parent(s) will authorize the represenative to travel with the child.
Choose "A child will travel abroad alone" if the child will travel abroad alone. Note that this should only be chosen if the parent(s) is confident that the child can travel alone.
Choose "A representative will accompany the child in applying for a passport" if the representative will only accompany the child in applying for a passport. In this case, the parent(s) or legal guardian will authorize the representative to apply for the child's passport.
Choose "A representative will accompany the child in applying for a passport and travelling abroad" if the representative will accompany the child in applying for a passport and travel abroad. In this case, the parent(s) or legal guardian will authorize the representative to travel with the child and to apply for the child's passport.
