Note that this document is specifically for a business or organization that wants to change its business name either before or after they have changed its business name. However, if individuals desire to change their name, they can use the Change of Name Notification.


How to use this document

This document is simple as it requires the old name of the business, the new name of the business, and the recipient.

After completing this document, it should be printed and signed by an officer of the business or another person. After signing the document, the letter should be sent to the recipient.

Alternatively, the sender can send this letter via email to the appropriate recipients. In this case, the sender should complete the form and forward it to the recipient's email address.

On the other hand, if the sender wants to apply for their business name change, this letter should be signed by either the director or company secretary and sent to the CAC together with the requisite CAC forms.
