The document provides a simple template that enables a person to give the other party a final chance to resolve a dispute before it is referred to a relevant ombudsman.

How to use this document

Once this document has been completed it can be posted to the other party. The sender can then wait for the other party to respond. If the other party does not respond within 14 days, the sender may wish to consider further action with the financial ombudsman.

The letter should be sent before the dispute is referred to the ombudsman.

In order to prove that the letter was sent, the sender may wish to send it by registered post, or keep a receipt or other proof of posting.

Applicable Law

The principles that will apply to this matter depend upon the ombudsman that covers the dispute. There are many different ombudsmen in Australia. Some are created under commonwealth legislation, and some are created under legislation at the state and territory level. Further information may be available from the office of the relevant ombudsman.

How to modify the template

You fill out a form. The document is created before your eyes as you respond to the questions.

At the end, you receive it in Word and PDF formats. You can modify it and reuse it.
